We accept most insurances with the exception of Celticare, Connecticare, and Rhode Island Medical Assistance.

For BMC plans, your primary care physician must be affiliated with Beth Israel Lahey Health.

Office visit co-pays are due the day of your visit. We accept cash, checks, debit and credit cards. After your claim has been submitted to your insurance company and they have paid their contracted amount, you will be billed for the remaining co-insurance or deductible. You can mail a check in the return envelope included with your statement.

If you wish to pay your balance by credit card, please call one of our offices with your account numbers, from 8 AM until 5 PM from Monday through Friday. Or, you may log into the Athena patient portal and pay with your credit card anytime day or night.

I was in a car accident and I need to bill my auto insurance or the other person’s insurance. What information do I need to schedule an appointment?2022-08-26T16:58:57+00:00

When you call for your appointment related to a motor vehicle accident, we will need the following information:

  • The claim number;
  • The name and address of the auto insurance company;
  • The claim adjustor’s full name, phone and fax numbers;
  • The date of injury;
  • Notes from another physician you man have seen regarding this accident along with any MRIs or X-rays.
I’m claiming Workers’ Compensation. What do I need to make an appointment?2022-08-26T16:57:03+00:00

When you Workers’ Compensation claim has been authorized, we will need the following information when you call to make an appointment:

  • Your claim number;
  • The name and address of the Workers’ Compensation insurance company;
  • The claim adjustor’s full name, phone and fax numbers;
  • The date of injury;
  • Notes from another physician you may have seen regarding this injury along with any MRIs or X-rays.

We are seeing many more health insurance plans that have yearly deductibles of $1,000, $2,500 or even $5,000 per year. To help you with your financial needs, we can set up a no-interest, monthly payment plan customized just for you. All you need to do is call one of our practice managers in Brookline (617) 232-2663 or Dedham (781) 326-1561 and they will create a plan to fit your budget.

Will my insurance carrier require a referral from my Primary Care Physician (PCP)?2022-08-26T16:51:57+00:00

The need for a referral varies by insurance policy:

  • If your insurance card says PPO on the front, you do not require a referral.
  • If your insurance cards says HMO on the front, you will need a referral from your PCP.
  • Masshealth patients require a referral from the PCP.
  • If you are a Medicare patient, they do not require a referral but you will need to have a Primary Care Physician (PCP).
  • For best results, patients should contact their insurance company to find out whether or not they need a referral to see an orthopedic specialist before calling our office for an appointment. The insurance company phone number will be printed on your insurance card.

Wheelchairs are located just inside the front door. If none are available when you arrive, please notify one of our staff.

In most cases, we can see you the SAME DAY. Call our office to make an appointment and let us know if you need immediate assistance. You will be evaluated in a shorter time than going to the emergency room! If you require surgery, we can usually schedule the procedure within a week or two.

We will try our best to accommodate you, but you may be asked to reschedule your appointment if you arrive late. If you need to reschedule for any reason prior to your appointment, please notify us at least 24 hours in advance.

In many cases, the answer is “yes.” Please call us to let us know you are interested in transferring your care. Request that all medical records pertaining to your surgery/treatment be sent to us. Our physicians will review your records. If transferring your care is a viable option, we will schedule an appointment for you.

Please bring any recent X-ray or MRI films that have been taken of the involved area. Your Primary Care Physician’s office may offer to send the images to us upon your request. Additionally, have with you

  • Any other tests or records pertaining to the problem area
  • Any necessary forms.
  • Complete insurance information
  • Any completed paperwork we may have mailed you (patient forms)
  • If you are under 18 years of age, a parent/guardian will need to accompany you unless prior arrangements have been made.
  • Must bring insurance card so that we can have the number on the front as well as the contact information on the back.
  • All WC information and MVA information including claim #s. We also need the claim representative’s name, address and phone number. for workman’s comp visits, we also need a contact name of the HR representative at your company.
Will I be charged a “facility fee” for an office visit?2022-08-24T18:26:05+00:00

No, Meeks and Zilberfarb Orthopedics does not charge an extra facility fee. When you visit health care providers at some area hospitals, you will receive two bills, one for the physician’s services and a second one, a facility fee, from the hospital which employs the doctor. Meeks and Zilberfarb Orthopedics and Sports Medicine is a private orthopedic practice and you will never be charged an additional facility fee for office visits.
